Objective/Purpose
Qualifications:
The Coordinator is responsible for establishing community connections to enhance activities and opportunities for DS clients. The Coordinator organizes, develops, implements, coordinates, supervises, and monitors the provision of services to DS clients in the community. The Coordinator will train and supervise staff that work with these individuals and will promote individual choice and meaningful services for each client.
Major Responsibilities
PROGRAM DEVELOPMENT: Develop a weekly schedule for DSPs and clients. Adjusts schedule as necessary for vacations, call outs, and additional DSP and client needs. In collaboration with the Manager, develop community activities and resources for program participants. The Coordinator will seek out new activities regularly enhancing client opportunities in the community. Activities should include leisure, educational, volunteering, health and wellness, social interactions, and more. Monthly client trips will be explored.
Ensure that all waiver services are scheduled and being received by everyone with community support hours, and any waiver changes are reported promptly to the Division Director. Review Monthly Summaries and data collection systems completed by the staff and make certain that services are provided as outlined in the individuals' Individual Service Agreement (ISA). Attends all Developmental Services and UCS meetings necessary to promote coordination and communication. Serves as facilitator for client ISA meetings with regards to community support goals and supports. Writes person centered client goals for ISAs. Facilitates client meetings for problem solving, changes in schedules, and other support necessities. Evaluate consumer goals for effectiveness and recommend changes to Case Manager/Qualified Developmental Disability Professional (QDDP) when needed. Train staff in methods and strategies needed to implement ISA outcomes and supports. Ensure that all regulations are adhered to. Develops individualized community connections for each individual which are reflective of the individual's ISA plan and ensures implementation. Ensures all staff are following regulations and addresses staff issues when they arise. Develops clear and meaningful expectations for all staff. Creates systems whereby all staff are appropriately trained on client support plans. Complete administrative duties as assigned.
Provide direct support when there are staff shortages. The Coordinator will be familiar with all community activities and be able to fill in when necessary. Participates in the DS on-call rotation as required.
SUPERVISION AND DEVELOPMENT OF PERSONNEL: Responsible for overseeing the Community Support Program. Provide direct supervision, support, and role-modeling to staff in the Connections Program. Schedule staff to ensure that client needs are addressed. Delegate responsibilities to staff as needed. Conduct annual performance evaluations per agency policy and ensure agency and program Key Performance Indicators (KPI's) are upheld. Assess training needs and ensure attendance at mandatory trainings, and schedule staff for additional training as needed. Conduct staff meetings and ensure open lines of communication is happening between staff and other parties, both inside and outside of the agency. Hires, trains, and onboards new staff. Assist the Manager with developing new training as new needs arise. Ensures program procedures and regulations are being followed.
Ensures that all staff are familiar with the division mission and philosophy that is being implemented across the program. Provides guidance and direction through the supervisory and evaluation process.
CLIENT ADVOCACY AND NETWORKING: Develops and distributes the monthly schedule of events and newsletter. Participates as a member of the Developmental Services Management team. Respond to requests for information and/or technical assistance as appropriate. Addresses client updates, issues, and information with families, home providers, and colleagues as needed and requested. Maintain contact with other team members on a regular basis. Monitor the quality of services provided to individuals. Effectively represent the agency to the public, employers, and to community members. Maximize the agency/division's reputation and cultivates support for agency services. Function as a representative of the agency as assigned. Serve on agency committees, as assigned.
CLINICAL RECORD KEEPING: Monitor time sheets and monthly progress notes to justify billing and decides when manager approval of monthly notes is no longer needed. Completes request for data as requested.
Required Qualifications
High School Diploma or equivalent with relevant experience required. Bachelor's degree preferred. Windows skills, both typing and navigating, and e-mail usage.
